Laser treatment methods
YAG - Laser capsulotomy.
After cataract surgery and implantation of an artificial lens, the lens bag begins to tighten after a while ( capsule ,that remains from the own lens), that is, secondary opacification of the capsular bag ,which leads to a decrease in visual acuity .
It is used to increase visual acuity and improve the quality of vision YAG-laser capsulotomy.
The technique consists in applying laser dot applications YAG laser on the cloudy posterior capsule , which leads to the formation of a rounded hole in the lens capsule , increases the access of light to the retina.
The procedure is painless and is performed on an outpatient basis. The doctor administers local anesthesia, which allows you to install a special lens in the eye to focus the rays on the back capsule of the lens. The patient sits in a comfortable position in front of the laser machine , an ophthalmologist places a lens in one eye, the other eye is fixed on some object at this time. Treatment takes less than half an hour, during which the patient can see green or red laser flashes.
After the procedure, visual acuity and intraocular pressure are monitored. To reduce inflammation, it is necessary to drip anti-inflammatory eye drops
In some cases LKT can be performed in two stages - in the case of too dense posterior capsule in order to prevent damage to the intraocular lens.
In the medical center "Laboratory of biomedical sensors and vision correction "Oko-Plus" YAG laser capsulotomy performed on the new Q-LAS laser model of the A.R.C.Laser company ( Germany), with wavelength 1064 nm , certified in Ukraine for use in laser treatment of secondary cataracts, pupillary membranes and vitreous membranes.
Laser trabeculoplasty
Laser trabeculoplasty it is used in glaucoma to reduce intraocular pressure and is indicated in open-angle glaucoma, pseudoexfoliative glaucoma and pigment-disperse glaucoma.
The technique consists in applying laser dot applications on the trabeculae ( drainage system) , which leads to improved outflow of aqueous moisture.
This procedure is performed when drug treatment is insufficiently effective as a supplement to drug treatment or to reduce the number of anti-glaucoma drops.
The procedure is painless and is performed on an outpatient basis. The doctor administers local anesthesia, which allows you to install a special lens in the eye (goniolinz), which has the ability to reflect rays. The patient takes a comfortable position in front of the lamp, an ophthalmologist places a lens in one eye, the other eye is fixed on some object at this time. Treatment takes less than half an hour, during which the patient can see green or red laser flashes.
After the procedure, intraocular pressure is monitored. To reduce inflammation, it is necessary to drip anti-inflammatory and anti-glaucoma eye drops - as prescribed by a doctor . After the procedure, blurred vision and increased sensitivity to light are possible, which persists for two days after the operation, no pain or discomfort should be felt.
After laser trabeculoplasty sometimes such complications occur:
- allergic manifestations (edema, itch, burning, redness of the mucous membrane);
- temporary increase in intraocular pressure (maybe with 1 by 7 a day after the procedure);
- lack of effectiveness of the procedure (observed approximately at 12% patients), which requires a surgical operation in the future.
Effectiveness of application laser trabeculoplasty , quite high, however, its action is not permanent: if the procedure is performed on one part of the trabecular network and this intervention was not sufficient, the doctor may need to perform it again, this time in a different part of the trabecular meshwork. There are cases, When, laser trabeculoplasty is not effective at all. After approx 2 years, in half of patients there is an increase in intraocular pressure to a dangerous level, which requires constant monitoring of intraocular pressure
In the medical center "Laboratory of Biomedical Sensory and Vision Correction "Oko-Plus" laser trabeculoplasty is performed on the new model of the CLASSIC laser of the company A.R.C.Laser ( Germany), with wavelength 532 nm , certified in Ukraine for use in laser treatment of glaucoma.
Laser iridotytomy
Laser iridotytomy used in closed-angle glaucoma or cases of closed angle of the anterior chamber in order to improve the outflow of aqueous humor and prevent an acute attack of glaucoma.
The technique consists in forming a hole in the peripheral part of the iris with a laser of a wavelength 1064 nm , which leads to improved outflow of aqueous moisture. This procedure is performed on paired eyes in patients with angle-closure glaucoma after an acute attack of glaucoma in one eye to prevent an acute attack in the other eye , and also in cases of narrow , coracoid and closed angle of the anterior chamber in order to prevent an acute attack of glaucoma.
The procedure is painless and is performed on an outpatient basis.
One hour before the laser intervention, the doctor drips anti-inflammatory drops and medically narrows the pupil with a pilocarpine solution 1 % . The doctor administers local anesthesia, which allows you to install a special lens for laser iridectomy in the eye. The patient is placed in a comfortable position in front of the laser, an ophthalmologist places a lens in one eye, the other eye is fixed on some object at this time. Treatment takes 15 -20 minutes, during which the patient can see green or red laser flashes.
In some cases, patients with a dark iris may require a two-stage intervention : 1 stage – local thinning of the place where the hole is formed by a wavelength laser 532 nm , 2 stage - passive laser iridectomy with a wavelength laser 1064 nm.
After the procedure, intraocular pressure is monitored. To reduce inflammation, it is necessary to drip anti-inflammatory eye drops and drops, which reduce intraocular pressure and dilate the pupil - as prescribed by a doctor . After the procedure, blurred vision and increased sensitivity to light are possible, which persists for two days after the operation, no pain or discomfort should be felt.
After laser iridotytomy sometimes such complications occur:
- the formation of local hemorrhages , which is associated with traumatization of the iris , that dissolve during the day;
- temporary increase in intraocular pressure (maybe with 1 by 7 a day after the procedure);
- inflammation of the iris , which is treated by instillation of anti-inflammatory drops;
- insufficient efficiency of the procedure (observed approximately at 12% patients), which requires a surgical operation in the future.
Effectiveness of application лазерної іридитомії , quite high, however, its action is not permanent and may require intervention in another segment of the iris. It is also necessary to remember, that the radical treatment of closed angle consists in removing the lens.
In the medical center "Laboratory of biomedical sensors and vision correction "Oko-Plus" laser Iriditomy performed on the new Q-LAS laser model of the A.R.C.Laser company ( Germany) with wavelength 1064 nm and the CLASSIC laser of the A.R.C.Laser company ( Germany), with wavelength 532 nm , certified in Ukraine for use in laser treatment of glaucoma.
Prophylactic peripheral laser coagulation
In eyes with myopia, areas of thinning and degeneration often appear in the retina. They arise due to the deterioration of the blood supply and nutrition of the peripheral parts of the retina.
Usually, the development of peripheral retinal degeneration is not accompanied by visual impairment and does not cause complaints, therefore, it is often detected by a doctor during an examination of the patient's retina.
Progressive thinning of the retina can lead to retinal detachment. For that, in order to avoid such a complication and to prevent surgery, preventive laser strengthening of the retina is performed using a special laser.
PPLK (prophylactic peripheral laser coagulation of the retina) is not performed to correct your vision, and in order to avoid complications of peripheral retinal degeneration — retinal detachment.
Strengthening of the retina can be limited - if dystrophic changes are present in limited areas of the retina, or circular - if degeneration zones are noted around the entire periphery of the retina.
The procedure is performed on an outpatient basis and does not require special training. Before the laser strengthening of the retina, the patient's pupils are maximally dilated with the help of special drops for that, to be able to examine and perform the procedure even on the most peripheral and hard-to-reach areas of the retina. Instillation of drops until the pupil is fully dilated takes 45 – 50 minutes. The procedure takes directly 10-15 minutes, but sometimes it is performed in several stages. The day after the event PPLK vision will be blurry, which is due to pupil dilation. In sunny weather, it is advisable to use sunglasses. The next day, you can return to your usual lifestyle.
Very rarely, a slight blurring of vision is noted after PPLC, which passes on its own and does not impair performance.
Carrying out preventive laser strengthening of the retina does not guarantee the absence of the possible development of other diseases of the retina and the possible further development of degenerative changes of the retina in other areas.
In the Medical Center "Ok-Plus” лазерне укріплення сітківки проводиться на новій моделі лазера CLASSIC компанії A.R.C.Laser ( Germany), certified in Ukraine for use in laser treatment of the retina.
Plk is performed by retinologists, who have the necessary experience in carrying out these procedures, which is confirmed by relevant certificates.
Panretinal laser coagulation of the retina
More than thirty years of experience using laser photocoagulation proves, that panretinal laser coagulation of the retina (PRLK) is the most effective method in the treatment of diabetic retinopathy and prevention of blindness. According to WHO recommendations, early detection and treatment with the help of a laser stops the progression of diabetic retinopathy and allows you to save vision for more than y 80% cases in patients with diabetes.
Laser photocoagulation is aimed at stopping the functioning of newly formed vessels, that contribute to the development of such severe changes in the organ of vision, as hemorrhages in the vitreous body, retinal detachment, secondary glaucoma.
It should be remembered, that panretinal laser coagulation is performed with a preventive purpose, to preserve vision and prevent blindness and is performed in several stages
To achieve a positive result of treatment, the patient needs to compensate for carbohydrate and lipid metabolism, control HbAc1 level and blood pressure, undergo regular ophthalmological specialized examination ,and with the progression of the disease and the appearance of new neoplasms – perform additional stages of laser coagulation.
The procedure is performed on an outpatient basis and does not require special training. Before performing PRLC of the retina, the patient's pupils are maximally dilated with the help of special drops . Instillation of drops until the pupil is fully dilated takes 45 – 50 minutes
After instillation of pain-relieving drops, a special contact lens is placed on the eye. The doctor precisely directs the laser beam to the areas of the fundus, that require treatment. The procedure takes 15 – 25 minutes and requires several stages (depending on the condition of the retina and the volume of the intervention). Sometimes after the procedure, an anti-inflammatory drug is injected to reduce the reaction of the retina to the laser. Your vision will be blurry the day after PRLC, which is due to pupil dilation. In sunny weather, it is advisable to use sunglasses.
The next day, the patient can return to his usual lifestyle.
In the medical center and "Laboratory of biomedical sensors and vision correction "Oko-Plus", laser treatment of the retina is carried out on the new model of the CLASSIC laser of the A.R.C.Laser company ( Germany), certified in Ukraine for use in laser treatment of the retina.
PRLK is performed by retinologists, who have the necessary experience in carrying out these procedures, which is confirmed by relevant certificates.
